Will my health insurance cover my visit?

Faith M. Gray, M.D. contracts with most health insurance plans. Prior to scheduling your appointment, you should check with your health insurance plan provider  to see if you have access to healthcare services at Faith M. Gray, M.D. and learn the specifics of your coverage. The customer service/member representative with your insurance company is the best person to inform you of the level of coverage you will have as a patient at Faith M. Gray, M.D. Otherwise, you may be responsible for all or a large portion of your bill. You can also find out what, if any, copayments, coinsurances and deductibles will be your responsibility.

Currently, Faith M. Gray, M.D. has contracts with the following major insurers and many others not listed:

What if I don't have insurance?

Short-Term convalescent care insurance plan**, Policy Forms: 4ST (or state variations)
(This is a limited benefit health insurance plan, which provides short term convalescent care. It is not intended to provide long term care benefits.)

  • Issue ages of 45-80
  • Shorter benefit periods than long-term care
  • Facilities covered include short-term convalescent care facility, assisted living and nursing home

Hospital and Acute Recovery Care Indemnity insurance plan**, Policy Forms:  4HP (or state variations).  Idaho Policy Form:  4HP(ID)

  • Issue ages of 45-80
  • Benefit period of 180 days per confinement
  • Facilities covered include hospital, assisted living or long term care

Life Insurance for Seniors, Policy Forms:  L9120F, CSA, CSB, L9200F, L9170F (or state variations).  Idaho Policy Form:  L900F12.

  • Level and graded death benefit available 
  • Simplified application
  • Competitive rates

Annuities, Policy Forms:  L7500F, L7505F, L7510F, L7800F, L7805F, L7810F, L7801F, L7806F, L7811F, L7850F, L7855F, L7860F (or state variations)
